NEW COVID-19 RULES & REGULATIONS • EFFECTIVE IMMEDIATELY

Please read completely and thoroughly to allow for a safe and efficient service.
Please ensure this waiver is completed prior to your scheduled appointment.

For the time being, service will NOT be performed on anyone who has, or who has been in contact with an individual who has had, or has tested positive for COVID-19, or is displaying symptoms of COVID-19. 

  1. Upon arrival to your appointment, please remain in your vehicle and call the salon at 239-603-3333 to notify us. We will call you as soon as your station has been properly cleaned and sanitized, and we are ready for you. For the time being, the waiting area will be closed.

  2. Upon entering the salon, you will be immediately instructed to wash your hands thoroughly prior to your service. There will also be a sanitation station available to you with hand sanitizer to use prior to, and during your appointment.

  3. Every guest will be required to have their temperature taken with a contactless forehead thermometer upon entering the salon.

  4. We have new plexiglass shields in place for each nail technician in order to ensure social distancing measures.

  5. We will be taking debit and credit card payments ONLY for the time being.

  6. ALL GUESTS AND STAFF will be required to wear a mask the entire duration of the service. Please bring your own, or you may purchase a mask at our front desk.

  7. NO ONE IS PERMITTED to accompany you during your service including spouses and children, unless they are receiving a service as well.

Please understand we are taking these precautionary measures for yours, and our staff members safety and security during this unprecedented time. If you are, or have been around someone who is ill or affected by the COVID-19 virus, we ask that you do not come in for the time being.

By agreeing to and submitting this form, you knowingly and willingly consent to have your service done during the COVID-19 pandemic and confirm that you are not presenting any of the following symptoms:

  1. Fever

  2. Shortness of breath

  3. Loss of sense of taste or smell

  4. Dry cough

  5. Runny nose

  6. Sore throat

You confirm that you have not been in contact with, or provided care to anyone who has tested positive, or is exhibiting one or more of the symptoms above.

You confirm you have not been out of state, or traveling in the last 14 days.
